Prairie du Chien, WI— Residents from Crawford and Clayton Counties are invited to attend a Community Health Needs Assessment focus group being held at the Prairie du Chien Public Library on Wednesday, October 20. The focus group will last one hour and will begin at 5:30 p.m. The first three participants to check in will receive a Kwik Trip gift card and a variety of other giveaway items will be available to all participants.
The conversation is being facilitated by staff from Crawford County UW Extension, in collaboration with Crossing Rivers Health, Gundersen Boscobel Area Hospital and Clinics, Grant Regional Health Center, Grant and Crawford County Health Departments, and Aging and Disability Resource Centers.
These organizations are working in partnership to help improve the health of their communities, and the region. Once gathered, this input will build on survey responses and statistical data shared from both Crawford and Grant County health departments. The results of this assessment will be used to develop strategies to address the prioritized needs, with the goal of contributing to improvements in the communities’ overall health.
The assessment framework includes:
Collecting and analyzing available data as well as feedback from online surveys and focus groups
Identifying gaps and needs and prioritizing those needs
Documenting and communicating results
Planning for action and monitoring progress
