Two people injured, one seriously in a two-vehicle accident in Fayette County

On May 14, 2026, at approximately 12:50pm, the Fayette County Sheriff's Office received multiple 911 calls of a two-vehicle accident at the intersection of 150th Street and R Avenue, approximately two miles west of Randalia. After further investigation, it was determined that Kaitlyn Marie Hackley, age 23 of Fayette, was traveling westbound in a Beige 2005 Ford Taurus SEL when she was struck by Joshua James Weltha, age 42 of Hawkeye. Weltha was operating a Gray 2007 Honda Pilot LX southbound and failed to obey the stop sign and yield right of way.

 

Hackley was trapped inside the Ford and mechanically extracted. Hackley received serious injuries and was transported to Gundersen Hospital in West Union. The Ford is considered a total loss.

 

Weltha received minor injuries and was transported to Sumner Hospital. The Honda is considered a total loss. The Honda had a total of three occupants. A male juvenile passenger received minor injuries and was transported to Sumner Hospital. A female passenger received life threatening injuries and was airlifted to May Clinic in Rochester, MN.

 

The Sheriff’s Office was assisted by the Hawkeye Fire Department, Hawkeye First Responders, Sumner Ambulance, Fayette Ambulance, Iowa Air Care, and Iowa State Patrol.

 

This accident remains under investigation.
