Crossing Rivers Hospice Program Discontinued

Prairie du Chien, WI — July 28, 2025 — Crossing Rivers Health has announced the decision to close its hospice program, effective September 30, 2025. The difficult decision, made by the organization’s leadership team and Board of Directors, follows a comprehensive evaluation of healthcare trends and reflects a commitment to responsible stewardship and long-term sustainability.
“Since the founding of our hospice program in 1980, our team has provided extraordinary care to patients and families across our service area,” said Chris Brophy, Chief Executive Officer at Crossing Rivers Health. “We have a deep respect for that legacy and are committed to supporting our patients, families, and dedicated hospice team throughout this transition.”
Over the past 45 years, the healthcare landscape and delivery of hospice care have evolved significantly. Today, hospice services are increasingly delivered by organizations that specialize exclusively in end-of-life care. In response to this shift, Crossing Rivers Health is focusing on meeting community needs through the organization’s core service offerings.
“We have a responsibility to make decisions that protect the future of our community-driven organization,” Brophy added. “By making this transition proactively—while we are in a position of stability—we are able to provide thoughtful support our patients, families, and staff through the change.”
As part of the transition, Crossing Rivers Health is working with Agrace Hospice Care in an effort to help ensure continuity of care for Wisconsin hospice patients and provide potential employment opportunities for affected hospice team members.
“As we evaluated hospice providers in our area, Agrace stood out for its mission, values, and nonprofit foundation, which closely align with ours,” said Brophy. “While we are encouraging our hospice staff to apply for internal positions here, we are also grateful to be able to offer additional potential employment options to our staff as well as ensuring a compassionate and seamless care transition for our Wisconsin patients—should they choose Agrace.”
For both Iowa and Wisconsin hospice patients, Crossing Rivers Health remains committed to working closely with each patient and their loved ones to facilitate a smooth transition to the hospice organization of their choice.
