Grant County Senior Farmers Market Program

The ADRC of Southwest WI office in Grant County will be distributing Senior Farmers Market Nutrition  Program Checks starting June 1. The Senior Farmers Market Nutrition Program (Senior FMNP) offers low income older citizens an opportunity to purchase fresh, locally grown fruits, vegetables, and herbs from  certified farmers. 

Senior FMNP checks can be used from June 1 through October 31 at any participating farmers market or  roadside stand in Wisconsin. In addition to offering nutritious foods, Senior FMNP supports local  economies by increasing the use of farmers' markets. 

Who Is Eligible? 

To be eligible to receive Senior FMNP checks, you must be at least 60 years old and meet income eligibility  requirements. Native Americans may participate at age 55 or older. In addition, the person must live in  Grant County.  

What do seniors get through Senior FMNP? 

Each qualified senior gets: 

• Five $5.00 checks that total $25.00 to spend between June 1 and October 31.  • Tips for using fresh, local produce.  

• A list of farmers' markets and farm stands where you can use your checks from June 1 through  October 31. 

Authorized Representative (or Proxy) 

1. An authorized representative (or proxy) may be designated to apply for certification and to purchase  SFMNP foods on behalf of the participant.  

2. A proxy can represent up to four (4) individuals. 

3. ADRC must obtain a signed statement from the eligible senior designating another individual as his/her  authorized representative. The senior must use the Eligibility Agreement form for this purpose. The senior  must designate who the proxy is, and both the proxy and senior must sign and date the form. This form  

must be completed prior to the proxy coming to the distribution site. 

  

 

ADRC will distribute the Farmers’ Market Checks at the designated locations and times and at the Aging &  Disability Resource Center office, 8820 Hwy 35/61 South, Community Services Building, Lancaster, WI,  Monday through Friday from 8:00 a.m. to 4:30 p.m. on a first–come, first–served basis. • June 13, Cuba City Meal Site - 11:00 – Noon 

• June 18, Cassville Meal Site 11:00 – 12:00 

• June 19, Montfort Meal Site 11:30 – 12:00 

• June 24, Lancaster Meal Site 11:30 – 12:00
